I have glanced over some SEO tools and they are SO expensive. Are they really important to do medical Search Engine Optimization or can I get decent rankings without them?

That's very true. Your SEO campaign will succeed or you will be able to do proper SEO only when you have access to the accurate data. Even though free tools can help you, you have limited access to data, so you might not be able to properly optimize your content or website. This does not mean everyone should buy expensive SEO tools, using free tools is a good start.
