Anytime I try to understand how medical SEO works, I feel overwhelmed. I know competitor analysis is very important, but have no idea how to pursue this? How do I know who is my competitor?

Use SEO tool like Semrush that will show you your competitors. But the easy way is the use Google search. Search for your target keywords on Google, pay attention to all the search results mostly on the first page. Those are your competitors.
